Valid Driver's License and access to a reliable vehicle is required for the Community Living Support Program. The DSP will help individuals stay active and engaged by providing transportation to fun community events, hobbies, appointments, shopping trips, and other outings / errands. They also assist with a variety of household duties including cleaning, laundry, meal prep, budgeting and establishing routines. Enrich and support the lives of the persons served while maintaining a safe and healthy environment. Provide companionship and personalized care while encouraging independence and self-care. Assist with activities of daily living, including community-based supports. Help the individual set and achieve goals; assist in overcoming personal barriers Communicate with clients and their families to understand their interests, skills, abilities, goals, and support needs. Implement positive behavior supports and de-escalate challenging behaviors when needed. Duties may periodically include personal care assistance and support with prescribed medications.
