This position plays a crucial role in providing direct nursing care and supporting safe, efficient clinic operations within a PACE geriatric clinic. The PACE Clinic Registered Nurse supports participant-centered care through clinic-based assessment, treatment support, education, vaccine and preventive health programs, supply readiness, documentation, interdisciplinary team collaboration, and cross-coverage for in-home nursing or nursing case management needs as indicated during program growth and scale. * Develop, implement, and coordinate individualized nursing care for PACE participants in the clinic setting using established procedures, protocols, and policies. * Conduct clinic-based nursing assessments, monitor changes in participant condition, coordinate follow-up needs, and support diagnostic testing and treatment plans as directed by the provider or interdisciplinary team. * Provide direct clinic nursing care, including vital signs, medication administration, wound care, injections, immunizations, treatment support, specimen collection, and assistance with medical procedures within scope of practice. * Work closely with physicians, nurse practitioners, clinic staff, rehabilitation staff, social workers, and other PACE team members to support comprehensive, coordinated, and compassionate participant care. * Educate participants and caregivers on treatments, medications, self-care, chronic disease management, vaccine recommendations, and prevention strategies to support participant engagement and health outcomes. * Support vaccine and preventive health programs, including vaccine planning, participant outreach, screening, administration, documentation, inventory coordination, and follow-u To view the full job description please use the link below. https://www.aplitrak.com/?adid=YmJnZW5lcmljLjQ3ODU0Ljg3NjRAcGFjaWZpY3NvdXJjZWNvbXAuYXBsaXRyYWsuY29t
