Ask for temporary open card OHP to keep your doctor

If you have a serious medical reason to see a provider outside your CCO, your provider can ask OHA for temporary open card (fee-for-service) coverage.

Steps

6 steps
  1. Talk to your CCO firstCall the customer service number on your member ID. Your CCO is your first contact whenever you need help getting care.
  2. Check that your provider takes open cardAsk if they accept open card OHP. If they only work with local CCOs, open card will not get you the care you need.
  3. Have your provider ask OHA for approvalTalk to the provider you want to keep seeing. They send OHA the forms and proof that you need temporary coverage to stay healthy.
  4. Give your provider the OHP 416 formThe OHP 416 is the Request for Temporary Fee-for-Service Coverage. Your provider also sends a written request and chart notes.
  5. Watch for the approval letterIf OHA approves, the letter says when coverage starts and ends. It lasts 6 to 12 months. Ask your provider for updates.
  6. Ask your provider to extend if neededIf you still need coverage after the approval period, your provider must send a new request to OHA.
