You can request an amendment to inaccurate or incomplete PeaceHealth records.

Steps

  1. Complete and sign the requestGive specific details of the information to correct, then sign the amendment form.
  2. Send the signed formSend it to PeaceHealth Health Information Management, Dept 336, 1115 SE 164th Ave, Vancouver WA 98683-9324, or fax 541-242-8046.
