Order your own Oregon driving record

You can order your own Oregon driving record online through DMV2U or by mailing DMV’s request form.

Steps

2 steps
  1. Verify identity for a mail requestUse the Order Your Own Record form for a mail request. Include enough identifying information, such as date and place of birth or address on DMV’s record.
  2. Sign and mail the request with paymentSign and date the request. Include a check or money order payable to Oregon DMV and mail to DMV Records Services, 1905 Lana Ave NE, Salem OR 97314.
