Replace a Consular Report of Birth Abroad

U.S. citizens can request a replacement Consular Report of Birth Abroad for themselves or their child born abroad.

Steps

3 steps
  1. Check who may request the recordYou must be the person named on the record and at least 18, a parent of a child under 18, an authorized government agency, or a person with written authorization from the named individual.
  2. Complete and notarize DS-5542Fill out DS-5542 by computer or in black ink and sign before a notary.
  3. Mail the request packetSend the form, photo-ID copy and payment to U.S. Department of State, Passport Vital Records, 44132 Mercure Circle, PO Box 1213, Sterling, VA 20166-1213.
