Request a Section 504 accommodation plan in Eugene 4J

Eugene 4J’s 504 team evaluates eligibility and determines whether a student needs an accommodation plan.

  1. Start with your school counselorContact the building counselor, usually the 504 case manager. After consultation, the case manager assistant will contact you for information and evaluation consent.
  2. Participate in the evaluation and team reviewProvide parent input and relevant medical documentation. The team also reviews school data and decides eligibility and any needed instructional or environmental accommodations.

    1. Have a parent or public agency make the referralReferrals may come from a parent or guardian or a public agency. Physicians, psychiatrists and counselors cannot refer directly.
    2. Contact the Special Education OfficeCall 541-790-7800 about the evaluation referral. A medical or psychiatric diagnosis alone does not establish eligibility.
