For people replacing ID or applying for SNAP, OHP, cash, or child care help.
- Start a benefits application at one.oregon.gov, call 800-699-9075 (weekdays 7 AM–6 PM), or visit an ODHS office. Eligibility depends on the program and household. Apply even if you lack proof of identity or income; ask the worker how to provide missing information.
- Gather what you have: household members, income and benefit letters, expenses, insurance information, and notices with deadlines. ODHS may request proof later. Save your application confirmation and respond to requests or interview appointments.
- For a state ID, review Oregon DMV’s accepted identity and residency documents before visiting: oregon.gov/odot/dmv/pages/driverid/idproof.aspx. Original or certified identity documents may be needed; a photocopy may not work. Standard ID and REAL ID have different requirements and fees.
- CCS may help replace an ID or birth certificate: 541-345-3628; Eugene 1464 W 6th Ave or Springfield 1025 G St. Ask whether funds are available, what documents you need, and how payment works before paying a fee.
If you cannot get help: If ONE online identity checking fails, use the phone or office application path. If you lack a stable mailing address, ask ODHS how to receive notices safely and ask a case worker about mail service; do not use a day center’s address without permission.
