Get an Oregon immunization record

Oregon offers several ways to request your family’s immunization records.

Steps

3 steps
  1. Ask a provider or schoolAsk your health-care provider or child’s provider for a record. A child’s school or child-care provider may also be able to print an ALERT IIS immunization history or Certificate of Immunizations Status.
  2. Submit an Oregon Immunization Program requestUse the Immunization Record Request Form, or a PDF or alternate-language form linked below it. If you use the online form, do not also submit a PDF. Follow the instructions for your selected request option.
  3. Account for incomplete recordsALERT IIS began collecting Oregon children’s data in 1996 and adult data in 2008; your age and places lived affect record completeness.
