  2. Choose whether it is confidentialThe form asks you to answer yes or no.
  3. Enter the violation address and detailsGive the violation address, use the location box only for a vacant lot, and describe the problem with specific detail.
  4. Attach photos or documentsYou can attach an image, PDF or Word document up to 8 MB.
